What to Expect at Your First Visit
Welcome and thank you for considering our office for your chiropractic care! To make your visit more efficient we have included our paperwork below:
Click HERE to complete your new patient paperwork. A tab with a “clipboard” will open in the browser. Please complete this on a laptop or desktop computer (the form will not transmit when completed on a mobile device). When completed, please click “submit” and the data will be securely transferred to our system. You do not need to complete every entry on the form. If something does not apply, simply skip that and go to the next question. At the end, it may say fields are incomplete, but it will transmit ok!
Auto Accident Form If you were in an auto accident, kindly print this form and bring it in with your visit instead of completing the online intake form. (You will need Adobe Acrobat Reader installed to access the paperwork. Click Here to Download Adobe Acrobat Reader for Free.)
The online intake is preferred, but we understand sometimes computers don’t do what we want them to. In the event you are unable to complete the online version, or if you simply prefer paper, please choose the appropriate intake form below, print out, and bring with for your visit!

We look forward to helping improve your health!
On your first visit to our chiropractic center you will be welcomed as a member of our family and our chiropractic assistant will greet you. You will then be asked to fill out our new patient forms which will help us gain some information about you and your condition.
Dr. Ranae Beard will then talk with you in comfortable surroundings and ask you some specific questions regarding your health and condition. Following a detailed consultation to determine if Gonstead chiropractic care may be of benefit to you, the doctor will let you know if she can help you. If Dr. Ranae determines she cannot help you, you will be referred to the appropriate health care provider.
After a thorough consultation with the doctor, you will have a complete chiropractic examination. This will include testing your reflexes, your ability to turn and bend as well as other standard orthopedic, neurologic, postural, and physical examinations. In addition, two computerized scans (surfaceEMG and thermography), are used to locate areas of swelling and inflammation along the spine. It is the latest in chiropractic examination technology. The doctor will gain valuable information regarding your condition and how to best help you. This information includes how to specifically reduce and correct spinal nerve interference caused by spinal subluxations so all parts of your body may communicate effectively as intended. If necessary, the doctor may take X-rays or refer you for additional testing procedures.
X-ray is sometimes required to get a complete evaluation of a patient. X-rays may be taken for several reasons. These include:
- to determine the condition of the discs (soft cushions between vertebrae)
- to determine the severity of spinal degeneration
- to locate problem areas in your spine such as: joint dysfunctions, misalignments, abnormal curves etc.
- to rule out other pathologies (disease). The need for X-ray is considered on a case by case basis. Our equipment is state of the art for imaging.
Report of Findings
Once all the information and examinations have been performed and the doctor has had ample time to study your results, your detailed exam will be explained and important questions will be addressed on your second visit to the office. Questions such as:
- What is my problem/condition?
- How long will it take to get fixed?
- How much will my care cost?
